Customising Enrolment Form Declarations


This article will assist you with customising the Declarations that appear on your Services Enrolment forms.


Overview


Xap's online enrolment form allows you to collect the personal information for the child and family.

The Enrolment form also contains a Declaration section allows you to capture the agreement from the Guardian that all the information collected on the enrolment form is True and Accurate.

There are default declarations on the enrolment form that you can choose to include as well as the ability to add your own custom declarations to support your services policies.


Note: Any new declarations that are added will send the Guardian an automatic Notification and Email to advise that **new declarations are required to be signed((.


Creating new Declarations


To create a new declaration to display on the enrolment form.


  1. Select the Settings screen
  2. Select Customise Enrolment Form icon



Note: Depending on your settings - Organisation, Provider or Centre, you may need to configure the changes individually.


  1. Select the Organisation, Provider or Centre name
  2. Select the Declaration section
  3. Select the +Add button
  4. Select Edit

  1. Create your custom declaration
Repeat to add additional declarations
  1. Select Save



  1. Select Save to save the changes to the Enrolment form.
