This article will assist you with customising the Declarations that appear on your Services Enrolment forms.
Overview
Xap's online enrolment form allows you to collect the personal information for the child and family.
The Enrolment form also contains a Declaration section allows you to capture the agreement from the Guardian that all the information collected on the enrolment form is True and Accurate.
There are default declarations on the enrolment form that you can choose to include as well as the ability to add your own custom declarations to support your services policies.
Creating new Declarations
To create a new declaration to display on the enrolment form.
- Select the Settings screen
- Select Customise Enrolment Form icon

- Select the Organisation, Provider or Centre name
- Select the Declaration section
- Select the +Add button
- Select Edit

7 Select the +Add button
- Create your custom declaration
- Select Save

- Select Save to save the changes to the Enrolment form.
