Children Health Conditions Report



When would I use this report?


The Children Health Conditions report will assist you with viewing and managing the details for children who have noted Health Conditions from their enrolment form within your Service.

The report will display all active and inactive children within your service and includes a unique identifier to allow you to cross-reference across multiple reports.



Where can you find this report?


  1. Select the Reports Screen
  2. Under the Family report section, select the Children Health Conditions Report



What will this report show?


The information displayed on this report includes:


Column

Heading

Description

A

Child

Displays the first and last name of the child

B

Known As

Displays the known as name if this has been set by the Guardian or Service (can be different to legal name).

C

Child DOB

Displays the DOB of the child.

D

Child Age-Mth

Displays the Age of the child in months.

E

Family

Displays the name of the Family the chid is linked to.

F

Child Status

Displays the active/inactive status of the child.

G

Health Condition

Displays the noted health conditions that have been flagged against the child eg. asthma, allergies.

H

Mgmt Plan signed

Displays the Y/N indicator if a management plan has been signed.

I

Mgmt Plan Exp Date

Displays the expiry date of the management plan.

J

Risk Mgmt Plan Signed

Displays the Y/N indicator if a risk management plan has been signed.

K

Risk Mgmt Plan Exp Date

Displays the expiry date of the risk management plan.

L

EpiPen/Anapen Exp Date

Displays the expiry date of the the epipen/anapen.

M

Medication

Displays the name of the medication that has been added for the child.

N

Health Comments

Displays any comments added regarding the noted health conditions for the child.

O

Other Comments

Displays any other comments added regarding the noted health conditions for the child.

P

Attachments

Displays any attachments that have been uploaded regarding the noted health conditions for the child.

Q

Rooms

Displays the Room/s the child is booked into.

R

Custom Fields

Displays the responses to any custom questions that have been added to the enrolment form under the health conditions section.

S

Family Status

Displays the active/inactive status of the family the child is linked to.

T

Centre

Displays the name of the centre the child is linked to.

U

Provider

Displays the name of the provider the child is linked to.

V

Organisation

Displays the name of the organisation the child is linked to.

W

Family ID

Displays the Family ID the child is linked to.

X

Child ID

Displays the ID of the child.

Y

Centre ID

Displays the ID of the centre the child is linked to.

Z

Organisation ID

Displays ID of the organisation the child is linked to.



How would I use this report?


The children report can be exported out to excel or csv formats as well as filtered.


Filtering the report


To refine the details listed,

  1. Select the reports option icon
  2. Select the filter icon from the drop-down


  1. You will then be able to view the current refinements showing. Select the section you wish to refine and edit the selection.
  2. Once complete, select apply.



To export the report,


  1. Select the reports option icon
  2. Select the export icon from the drop-down
  3. Select the report format either excel or csv
